Picture a household, a small number of residents, their families and a small number of permanently assigned staff – sitting down as equals, planning, deciding, doing, working, enjoying life together.

Quality of life for our residents is at the center of everything we do.  All elders, no matter where they live, have a right to control and choice, aging in place, purpose and meaning, and the pleasures of living that we all seek.

Perham Memorial Skilled Care Community is a 96-bed, 24-hour a day, skilled care facility that embraces PersonFirst™ values and the Household Model.  We genuinely believe that decision-making should begin with the resident.  Our responsibility is to support and encourage the resident to live his life as he chooses and to adapt to his routine by placing the person before the task.  The resident’s daily routine becomes the plan of care with staff supporting the resident in the life he has chosen to live within our facility.

In October 2006, we opened the doors to our new building.  Six households all with a kitchen, dining room, living room, private dining room, fireplace, and porch are home to 16 residents each.  Caregivers cross-trained in all areas and certified as nursing assistants respond quickly to resident needs, no matter what they are. 

The Town Center is the community within our broader community.  Residents can “go to town” and enjoy a movie at the theatre, stop at the barber/beauty shop for a hair cut, shop, go to church, have coffee in the Courtyard Café, or enjoy activities with children in the schoolhouse. 

The Town Center overlooks our beautiful Courtyard of Caring – a garden with stunning summer color and plants that bring back memories and invite birds and butterflies.  Raised bed planters for flowers and vegetables accommodate gardeners standing or in wheelchairs and encourage garden activities.  Paved walkways enable residents to stroll with ease throughout the Courtyard.  Views from the households are spectacular – even in the winter months as plants provide shelter and seeds for birds and hold snow and frost for winter interest.
