Perham Memorial Hospital and Home (PMHH), located in Perham, Minnesota, is seeking submissions for artwork in the Healing Garden at the new hospital to create a healing environment that supports patient- and family-centered care and recognizes the founding donors of the Together – Building for the Future capital campaign
About PMHH
- Located in Otter Tail County in west central Minnesota
- Owned by the Perham Hospital District which consists of three cities and 10 townships
- A new 120,000 square foot hospital/clinic is under construction; scheduled to open January 2012
- 25-bed Critical Access hospital; 96-bed skilled care long-term facility located just blocks from the new hospital
- Provides medical, surgical, chiropractic, obstetrical, rehabilitation, and newborn care
- Employees 420 people
- 160 volunteers
PMHH has a preference to select art work from artists who live in the Perham and the lakes region area. While works of art from regional artists is highly desired, works of art from other geographical areas will be considered. Collaborative works of art are welcomed.
Description of art being sought
A piece of artwork that will be displayed in the Healing Garden, approximately 15 x 10 feet and constructed of metal, three-dimensional, and will artfully depict flowers, plants, grasses, and animals native to the area. The base of the sculpture will be made of a material that can be engraved with the names of donors to the new hospital capital campaign. See the attached rendering for location of the artwork. The new hospital floor plan and artistic renderings of the new hospital are available to review on the PMHH website.
What to submit
1. Letter of interest about your approach to the project.
2. Professional resume including information regarding past public art commissions, design team experience, awards, grants, and education if applicable.
3. Three professional references.
4. Digital images (jpegs) of past work with annotation on a compact disk.
